Source Identifier: RV-003
1. (S/NF/SK) TASKING: Target was the second floor of Area # 2 . ( i.e . , the floor above
ground level) . Some was directed to use attached layout sketch for a worksheet.
2. (S/NF/SK) SESSION : Monitor was based by cav 1 -4 . however had considered Source RV-02 1
( who first worked this target in CRV-4) to have been mostly " off ( signal ) line."
Consequently, monitor experienced a great deal of suprise when RV-003 began to
report basically the "some information regarding the largest .
Session began, and remained, in Stage 6 throughout session.
Source was dividing to " isolate" infor pertaining to the records he perceived ,
as well as to 55/ 56 " subjects , for whom the records were kit .
2. (S/NF/SK) SUMMARY: 2d floor, area # 2; two separate areas where described by
source - one area was controlled access records room in which were
kept files on " subjects " That were undergoing a " procedure " in another
( closed) part of the same floor. These subjects were placed inside

Very detailed records were kept on the subjects
with reference to their "various"states and conditions "
records were described as " clinical" in nature.
Source felt that "subject were carefully selected
and once in the system", their whereabouts ito,
was kept hidden.

(S/NF/SK) FEEDBACK - Source was told by monitor that his
infor was very similiani to that of RV-021 when on this
target.
